La Tavola Online Donation Request Form

We strive to make a difference in the communities we serve, and feel blessed to have the opportunity to touch people’s lives - to pay it forward! Of course we realize that all charities intend to serve worthwhile causes - but, because we receive a significant number of requests each month, and in order for us to make a real impact, we have chosen to lend our support exclusively to children and to AIDS related events.

Once a month, our "From the Heart" committee reviews all requests submitted. In fairness to all, we can only consider those requests made using this form; we cannot handle requests made in person or over the phone. Please know that all requests will be given serious consideration, and - while we will respond to each request - please allow up to 6 weeks for our response.
